Sutter Health Advanced Practice Pharmacist - Oncology in Palo Alto, California

Position Overview:

Will initiate, modify and discontinue medications within a Collaborative Practice Agreement as defined by the California State Board of Pharmacy. Focus on management of complex disease states to help patients reach optimal therapeutic goals. Simplify patient medication regimens, minimize the risk of harmful medication interactions and educate patients to enhance adherence and clinical outcomes. Responsible for policy development, Continuous process improvement, continuing education, clinical and medical staff development and Pharmacy APPE student and resident preceptorship. Performs patient care assessments, orders and interprets laboratory tests, prescribes, modifies and discontinues medication therapies. Monitors clinical outcomes, communicates with medical providers, performs electronic documentation and refers patients to other health care providers.

Preferred Criteria:

  • Experience as a specialty pharmacist in in a hospital setting.

  • Board Certification (ex: BCOP/BCACP/BCPS)

  • EPIC and Beacon experience.

Job Summary/Responsibilities:

  • This position requires flexibility to orient and work at provider clinic and specialty pharmacies.

  • Clinical knowledge and therapy management for Oncology diseases states.

  • Maintain excellent written and verbal communication skills, strong critical thinking skills.

  • Excellent organizational skills, ability to prioritize/follow-up.

  • Flexible; able to cope with rapid change; comfortable with an ambiguous environment.

  • Ability to troubleshoot problems efficiently and effectively.

  • Ability to provide comprehensive medication management including initial assessments, toxicity evaluations/symptom management and reassessments.

  • Capability to set priorities amongst a series of competing clinical and operational priorities and to effectively coordinate and direct the work of others to meet specified deadlines.

  • Proficient computer experience and accurate data entry skills in Word and Excel.

Pay Range is $97.33 to $111.92 / hour

The salary range for this role may vary above or below the posted range as determined by location. This range has not been adjusted for any specific geographic differential applicable by area where the position may be filled. Compensation takes into account several factors including but not limited to a candidate’s experience, education, skills, licensure and certifications, department equity, training and organizational needs.
