The role you’ll contribute:

The Structural Heart RN Coordinator is responsible for the management and success of the Structural Heart Program. Structural heart procedures include TAVR, MitraClip and Left Atrial Appendage Occlusion (LAAO). The Structural Heart RN Coordinator will be responsible for coordinating the patients through the selection process for structural heart therapy, from initial referral through one year follow-up. This would include working with the Cardiac Surgeons, Interventional Cardiologists, Anesthesiologists, and others on the Structural Heart Program team.

PRINCIPAL DUTIES AND JOB RESPONSIBILITIES :

  • Coordinate all aspects of the structural heart program including; TAVR, LAAO, and Mitra Clip.

  • Coordinates overall Structural Heart Program ensuring processes and protocols are in place to assure smooth operations and patient flow.

  • Manages all incoming referrals including but not limited to:

  • Oversite of initial demographic and clinical evaluations on patient referrals.

  • Ensure completion of appropriate orders, scheduling, and appropriate screening tests (Ie: laboratory, radiology, echo, PFT and other studies)

  • Compile full screening portfolio for each screened patient for physician team review at the multi-disciplinary valve conferences.

  • Initiate consultations and referrals as dictated by the physician team.

  • Schedule procedures

  • Ensure consistent and appropriate follow-up care in collaboration with referring and or primary care physicians.

  • Manage the full patient experience from referral to procedure. Including but not limited to:

  • Educate the patient and family on the referral and screening process.

  • Maintain consistent communication with the patient and family throughout the screening and pre-procedure process.

  • Follow up with patient via phone post procedure to check in on status and needs.

  • Promote understanding of the underlying disease process and care planning.

  • Work collaboratively with the vendors for the program to ensure compliance with all requirements (ie: screening tests loaded into the review system. Preparation work complete for any proctored cases etc.)

  • Complete all documentation to ensure compliance with regulatory standards related to quality and billing (ie., complete appropriate registry data collection for all patients, etc.)

  • Ensure uniform and comprehensive clinical data collection per CMS and all regulatory guidelines.

  • Assure all documentation and billing requirements are met per CMS and all regulatory guidelines.

  • Work with marketing team as needed to develop educational materials and consistent collateral for the Structural Heart Program.

  • All other duties as assigned.

Qualifications - External

The expertise and experiences you’ll need to succeed:

EDUCATION AND EXPERIENCE REQUIRED:

  • Baccalaureate degree from an accredited RN program.

  • Minimum five years clinical experience specific to Cardiology.

EDUCATION AND EXPERIENCE PREFERRED:

  • Three years acute cardiac care.

LICENSURE, CERTIFICATION OR REGISTRATION REQUIRED:

  • Current Georgia RN License
